Understanding BMI: Formula, Categories, Limitations, and Alternatives

Body Mass Index (BMI) is a screening tool that estimates body fat based on height and weight. Developed by Belgian mathematician Adolphe Quetelet in 1832, it remains one of the most widely used health metrics despite well-known limitations.

Known Limitations of BMI

BMI is a population-level screening tool, not a diagnostic measure. It has significant limitations:

• Ignores body composition: A muscular athlete and a sedentary person can have the same BMI. Muscle weighs more than fat by volume.
• Age differences: Older adults naturally lose muscle mass, so a "normal" BMI may still indicate excess body fat.
• Ethnic variations: Health risks vary at different BMI levels across ethnic groups. Asian populations may face higher risks at lower BMI values.
• No fat distribution data: Visceral fat (around organs) is far more dangerous than subcutaneous fat. BMI cannot distinguish between them.

Better Alternatives and Complementary Metrics

• Waist circumference: Men above 40 inches (102 cm) and women above 35 inches (88 cm) have higher health risks, regardless of BMI.
• Waist-to-hip ratio: Dividing waist circumference by hip circumference. Above 0.90 (men) or 0.85 (women) indicates increased risk.
• Body fat percentage: Measured via calipers, DEXA scan, or bioelectrical impedance. Gives a direct measure of fat vs. lean mass.
• Waist-to-height ratio: Your waist should ideally be less than half your height. Simple and effective.

When BMI Is Still Useful

Despite its limitations, BMI is useful for large-scale population health studies and as a quick initial screening. It is fast, free, and requires no special equipment.
